# NPI Registry Batch Lookup and Normalizer (`anderson-data-tools/glacier-lean-4`) Actor

Batch lookup exact NPI numbers in the official CMS NPPES registry. Normalize provider names, primary taxonomy and practice locations with dated source links. No-match and unavailable-source results are explicit; this is not credentialing verification.

## Pricing

$3.00 / 1,000 provider records

This Actor is paid per event. You are not charged for the Apify platform usage, but only a fixed price for specific events.

## What's an Apify Actor?

An Actor is a serverless cloud program that runs on the Apify platform. It has two run modes.
In Batch mode, an Actor accepts a well-defined JSON input, performs an action which can take anything from a few seconds to a few hours,
and optionally produces a well-defined JSON output, datasets with results, or files in key-value store.
In Standby mode, an Actor provides a web server which can be used as a website, API, or an MCP server.

Apify vocabulary and the platform model are defined once, in the agent quickstart at https://apify.com/agents.md.

## How to integrate an Actor?

If asked about integration, you help developers integrate Actors into their projects.
You adapt to their stack and deliver integrations that are safe, well-documented, and production-ready.

Do not guess an integration path. Every one of them is in the agent quickstart at https://apify.com/agents.md: the Apify MCP server, Agent Skills with the Apify CLI, the JavaScript and Python clients, the REST API, and the account-free path for an agent with no human to sign in. It also carries the rule on stating cost before the first paid run.

For examples already wired to this Actor's own input schema, see the [API](#api) section below.

Each client library has reference documentation the quickstart does not restate: [JavaScript/TypeScript](https://docs.apify.com/api/client/js/docs.md) (`npm install apify-client`) and [Python](https://docs.apify.com/api/client/python/docs.md) (`pip install apify-client`).

# README

## NPI Registry Batch Lookup and Normalizer

Look up exact NPI numbers in the official CMS NPPES registry. Normalize provider name, primary taxonomy, practice location, and source dates with explicit no-match and uncertainty.

### Input example

```json
{
  "npis": ["1881018208"]
}
```

Supply 1–50 ten-digit NPIs as strings. The Actor checks each check digit before making requests and queries duplicate values only once. It does not search by provider name.

### Output example

Selected fields from a recorded local SDK result on October 11, 2026 UTC. NPPES may update a record later; this sample is not a current credential or practice-status guarantee.

```json
{
  "record_id": "1881018208",
  "npi": "1881018208",
  "outcome": "ok",
  "billable": true,
  "message": "Public NPPES registry record only; not license, exclusion or credential verification.",
  "name": "MAYO CLINIC",
  "enumeration_type": "NPI-2",
  "registry_status": "A",
  "source_updated_at": "2021-04-12",
  "enumeration_date": "2014-02-05",
  "taxonomy_code": "261QM1300X",
  "taxonomy_description": "Clinic/Center, Multi-Specialty",
  "city": "ROCHESTER",
  "state": "MN",
  "postal_code": "559050001",
  "source_url": "https://npiregistry.cms.hhs.gov/api/?version=2.1&number=1881018208",
  "retrieved_at": "2026-10-11T03:03:41.402163+00:00"
}
```

Each returned NPI is one dataset row. The Actor selects the practice `LOCATION` address and a unique primary taxonomy when present. Missing optional values stay null with a warning. Source hashes, URLs, source dates, and retrieval times support checking the returned record.

### Result meaning and limits

`ok` means the exact NPI was returned by the registry. `no_match` means that query returned no public record; the message names NPPES and the retrieval date. It is free and is not proof that an NPI is invalid. `uncertain` means the response was unavailable, malformed, or did not match the requested identity; its message begins `uncertain - please check` and it is free. A valid check digit only verifies the number's format.

Uses the [CMS NPPES NPI Registry API](https://npiregistry.cms.hhs.gov/api-page). Registry presence is not licensure, credentialing, exclusion, Medicare participation, or permission to contact a provider. The Actor excludes phone/fax, authorized officials, and full source records. Never submit patient data. No accuracy rate or CMS endorsement is claimed; verify important facts with CMS.

### Pricing

The `provider-record` event costs **$0.003 per returned provider**, with platform usage included. No-match and uncertain results are free.

The maximum event charge per run is **$0.15** for 50 input NPIs. Fewer returned providers cost up to $0.003 each. Each new run can incur new event charges.

# Actor input Schema

## `npis` (type: `array`):

1–50 ten-digit NPI strings with valid check digits. Duplicates are looked up once. Registry presence is not credentialing.

## Actor input object example

```json
{
  "npis": [
    "1881018208"
  ]
}
```

## `results` (type: `string`):

You can run this Actor programmatically using our API. Below are code examples in JavaScript, Python, and CLI, as well as the OpenAPI specification and MCP server setup.

## JavaScript example

```javascript
import { ApifyClient } from 'apify-client';
// Initialize the ApifyClient with your Apify API token
// Replace the '<YOUR_API_TOKEN>' with your token
const client = new ApifyClient({
    token: '<YOUR_API_TOKEN>',
});
// Prepare Actor input
const input = {
    "npis": [
        "1881018208"
    ]
};
// Run the Actor and wait for it to finish
const run = await client.actor("anderson-data-tools/glacier-lean-4").call(input);
// Fetch and print Actor results from the run's dataset (if any)
console.log('Results from dataset');
console.log(`💾 Check your data here: https://console.apify.com/storage/datasets/${run.defaultDatasetId}`);
const { items } = await client.dataset(run.defaultDatasetId).listItems();
items.forEach((item) => {
    console.dir(item);
});
