- Initial release: `search`, `lookup`, `monitor` and `safety` modes over the FMCSA / DOT open
  data on `data.transportation.gov` (Socrata, keyless).
- Joins the carrier census with the SMS BASIC safety measures, authority history, revocation
  orders, active and historical insurance filings, the crash file, the inspection file and
  out-of-service orders - by USDOT number and MC docket number.
- `includeSafety` adds the seven CSA BASIC measures with alert flags and driver / vehicle
  out-of-service rates plus a 24-month crash summary; `includeHistory` adds the authority
  lifecycle and every insurance filing; `includeInspections` adds per-inspection and per-crash
  detail. Each is billed as a separate event, only when it produced data.
- `monitor` mode returns carriers with a new registration, authority revocation / reinstatement,
  insurance lapse or out-of-service order since a date, each tagged with the change type.
- `search` filters: state, operation class, cargo type, hazmat, safety rating, fleet-size range,
  registration-date window, name keyword.
- Look-up accepts USDOT numbers, MC/MX docket numbers and company names; ids that do not resolve
  write an `error: "not found"` row.
