## What's an Apify Actor?

An Actor is a serverless cloud program that runs on the Apify platform. It has two run modes.
In Batch mode, an Actor accepts a well-defined JSON input, performs an action which can take anything from a few seconds to a few hours,
and optionally produces a well-defined JSON output, datasets with results, or files in key-value store.
In Standby mode, an Actor provides a web server which can be used as a website, API, or an MCP server.

Apify vocabulary and the platform model are defined once, in the agent quickstart at https://apify.com/agents.md.

## How to integrate an Actor?

If asked about integration, you help developers integrate Actors into their projects.
You adapt to their stack and deliver integrations that are safe, well-documented, and production-ready.

Do not guess an integration path. Every one of them is in the agent quickstart at https://apify.com/agents.md: the Apify MCP server, Agent Skills with the Apify CLI, the JavaScript and Python clients, the REST API, and the account-free path for an agent with no human to sign in. It also carries the rule on stating cost before the first paid run.

For examples already wired to this Actor's own input schema, see the [API](#api) section below.

Each client library has reference documentation the quickstart does not restate: [JavaScript/TypeScript](https://docs.apify.com/api/client/js/docs.md) (`npm install apify-client`) and [Python](https://docs.apify.com/api/client/python/docs.md) (`pip install apify-client`).

## NPI Provider Search: US Healthcare Providers from the NPI Registry

**$1 per 1,000 providers. Platform usage included. No rental, no minimum.**

The four most used NPI registry actors on Apify Store charge $2.00 to $5.00 per 1,000 providers (Store prices checked 9 October 2026). This actor returns the same kind of data for $1.

[Open it in Apify Console](https://console.apify.com/actors/CWmzlDQ0dvbNWIFPi/input)

### What it costs

| Providers | This actor | Most used NPI registry actors on the Store |
|---|---|---|
| 1,000 | $1.00 | $2.00 to $5.00 |
| 10,000 | $10.00 | $20 to $50 |
| 100,000 | $100.00 | $200 to $500 |

Plus $0.001 per run. A weekly pull of 2,000 dentists costs about $2.00 per run, or about $8 a month.

### Why people switch

- **One flat price.** Compute is included, so the bill follows the number of providers and nothing else. The registry needs no proxy and no key.
- **More than the registry's own 1,200 limit.** One search in the NPI registry stops at 1,200 records. When a search is bigger, the actor splits it by provider type, state and ZIP prefix, and drops repeats. A test run for dentists in Texas returned 3,000 distinct providers.
- **Full records.** Every taxonomy with its license number and state, other practice locations, other identifiers, the authorized official of an organization, and published Direct messaging addresses.
- **Lists of NPI numbers.** Paste your own list and get one record per number in the same run.

### Quick start

1. Open the actor, type a specialty such as `Cardiovascular Disease`, pick a state, press **Start**.
2. Or paste this input:

```json
{ "specialties": ["Cardiovascular Disease"], "states": ["NY"], "maxItems": 200 }
```

3. Download the results as JSON, CSV or Excel, or read them through the API.

### Use it from code

```bash
curl -X POST "https://api.apify.com/v2/acts/one_dollar_store~npi-providers-search/run-sync-get-dataset-items?token=YOUR_APIFY_TOKEN" \
  -H "Content-Type: application/json" \
  -d '{"specialties": ["Cardiovascular Disease"], "states": ["NY"], "maxItems": 100}'
```

```python
from apify_client import ApifyClient
client = ApifyClient("YOUR_APIFY_TOKEN")
run = client.actor("one_dollar_store/npi-providers-search").call(
    run_input={"specialties": ["Cardiovascular Disease"], "states": ["NY"], "maxItems": 100}
)
for item in client.dataset(run["defaultDatasetId"]).iterate_items():
    print(item["npi"], item["name"], item["city"], item["phone"])
```

### Use it from an AI assistant

Claude, ChatGPT, Cursor and other assistants can call this actor through Apify's MCP server. Add it with this server address:

```
https://mcp.apify.com?tools=one_dollar_store/npi-providers-search
```

Then ask, for example: "List 50 dentists in Austin, Texas with their NPI numbers and phone numbers."

### Switching from another NPI registry actor

Most NPI actors take a name or specialty, a state and a result limit. Here they are `specialties` and `lastName` or `organizationName`, `states` and `maxItems`. The core output fields are `npi`, `name`, `providerType`, `specialty`, `city`, `state` and `phone`.

### Input

| Field | Type | Default | What it does |
|---|---|---|---|
| `specialties` | list of text | none | Words from the specialty (taxonomy) description, for example `Cardiovascular Disease`, `Dentist`, `Family Medicine`. Each one is searched on its own. |
| `states` | list | none | Two letter state or territory codes. Needs a specialty, name, city or ZIP next to it. Empty means the whole country. |
| `firstName`, `lastName` | text | none | Individual providers. Common nicknames also match. End with `*` to match names that start with your text. |
| `organizationName` | text | none | Clinics, hospitals, pharmacies, groups. Matched as typed, so end with `*` for names that start with your text, for example `mayo*`. |
| `city` | text | none | City as the registry spells it. |
| `postalCode` | text | none | A 5 digit ZIP code, or the first digits with a star, for example `100*`. |
| `npiNumbers` | list of text | none | Look up providers by 10 digit NPI number. Works alone or next to a search. |
| `providerType` | text | `any` | `any`, `NPI-1` (individuals) or `NPI-2` (organizations). |
| `addressType` | text | `LOCATION` | What state, city and ZIP are matched against: `LOCATION` (practice locations), `PRIMARY` (primary practice location only), `MAILING` or `ANY`. |
| `maxItems` | number | 100 | Total providers for the run. Minimum 10. |

Give at least one of specialty, name, organization, city, ZIP or NPI numbers. The registry does not accept a state on its own. Example:

```json
{
  "specialties": ["Dentist", "Orthodontist"],
  "states": ["TX", "OK"],
  "providerType": "NPI-1",
  "addressType": "PRIMARY",
  "maxItems": 5000
}
```

### Output fields

| Field | Meaning |
|---|---|
| `npi` | The 10 digit National Provider Identifier. |
| `providerType` | `Individual` or `Organization`. |
| `name` | Full name of a person, or the organization name, in capitals as the registry stores it. |
| `firstName`, `middleName`, `lastName`, `namePrefix`, `nameSuffix`, `credential`, `gender`, `soleProprietor` | Individuals. |
| `organizationName`, `organizationSubpart`, `authorizedOfficial` | Organizations: the name, whether it is a subpart of another, and the official's name, title and phone. |
| `status`, `enumerationDate`, `lastUpdated`, `certificationDate` | Registration status and dates. |
| `specialty`, `taxonomyCode`, `licenseNumber`, `licenseState` | The primary specialty and its license. |
| `taxonomies` | Every specialty: code, description, primary flag, license number and state. |
| `address1`, `address2`, `city`, `state`, `postalCode`, `country`, `phone`, `fax` | Practice location. |
| `mailingAddress` | Mailing address, same layout. |
| `otherPracticeLocations`, `otherNames`, `identifiers`, `endpoints` | Extra locations, other names (for example doing business as), other identifiers such as Medicaid numbers, and published Direct messaging addresses. |
| `registryUrl`, `source`, `scrapedAt` | Link to the registry page, data source and collection time. |

Fields the registry leaves empty are left out of the row.

]
```
### Limits
- **One registry search holds at most 1,200 providers.** Bigger requests are split by provider type, then state, then ZIP prefix, and repeats are dropped by NPI number. If a single 5 digit ZIP code still has more than 1,200 matches, the rest of that ZIP is not returned and the log says so.
- **Use the registry's own specialty words.** Cardiologists are listed as `Internal Medicine, Cardiovascular Disease`. A search for `Cardiology` finds a different group (pharmacists with a cardiology specialty).
- **Order.** Rows come in the order the registry returns them, roughly alphabetical by name, not ranked by relevance.
- **Pace.** A test run delivered 3,000 providers in about 270 seconds, roughly 10 per second. A run of 30,000 takes close to an hour, so raise the run timeout for very large pulls or split them by state.
- **No general email or website field.** The registry does not publish them. A Direct messaging address appears in `endpoints` only when the provider registered one.
- **What providers reported.** The data is what each provider told the registry. The actor does not check licenses or whether a practice is still open.
### FAQ
**What is the cheapest NPI registry scraper on Apify?**
This actor costs $1 per 1,000 providers with platform usage included. The most used NPI registry actors on the Store charged $2.00 to $5.00 per 1,000 when we checked on 9 October 2026.
**How much does it cost to scrape 10,000 NPI records?**
$10, plus $0.001 for each run.
**Do I pay for compute or proxy on top?**
No. Platform usage is included in the $1 per 1,000 price. The registry is read directly, so no proxy is used.
**Where does the data come from?**
From the official NPI Registry API published by the US Centers for Medicare & Medicaid Services (CMS). The actor calls the same public endpoint a browser would, at a modest pace. No key and no login are needed.
**How do I get every dentist in a state?**
Set `specialties` to `["Dentist"]` and `states` to your state, and raise `maxItems`. If the state has more than 1,200 matches, the actor splits the search by provider type and ZIP prefix on its own.
**How do I look up a list of NPI numbers?**
Put them in `npiNumbers`, one per line. Each number returns one record.
**Is it legal?**
The actor reads public data only, with no login. The NPI Registry is a public directory that the US government publishes for anyone to use. How you use the data is up to you: follow the registry's terms, marketing and privacy rules such as TCPA and CAN-SPAM, and the law where you live.
**Is this an official CMS or NPI Registry product?**
No. It is independent and not affiliated with or endorsed by CMS or the US government.
**Do I need an Apify account?**
Yes. Actors run from an Apify account, and a free account is enough to start: it includes $5 of platform usage every month. You can [sign up here](https://www.apify.com?fpr=ymnoit). This is a referral link and it costs you nothing.
**Is there a limit on the free plan?**
Yes. A run started from a free plan account delivers the first 10 results, then ends with a message and a link to the plans. Any paid plan returns everything you ask for, up to your own limit. The price per result is the same on every plan.
### Changelog
- October 2026: first release.
# Actor input Schema
## `specialties` (type: `array`):
Words from the provider's specialty (taxonomy) description, one per line, for example Cardiovascular Disease, Dentist, Family Medicine, Pharmacist. Each one is searched separately.
## `states` (type: `array`):
Only providers in these states or territories. Needs a specialty, name, city or ZIP code next to it, because the registry does not accept a state on its own. Leave empty for the whole country.
## `firstName` (type: `string`):
Individual providers. Common nicknames also match, as in the registry. End with \* to match names that start with your text (at least 2 letters before the \*).
## `lastName` (type: `string`):
Individual providers. End with \* to match names that start with your text, for example sm\*.
## `organizationName` (type: `string`):
Clinics, hospitals, group practices, pharmacies and other organizations. The registry matches the name as typed, so end with \* to match names that start with your text, for example mayo\*.
## `city` (type: `string`):
City name as the registry spells it, for example Buffalo.
## `postalCode` (type: `string`):
A 5 digit ZIP code, or the first digits with a star for an area, for example 10036 or 100\*.
## `npiNumbers` (type: `array`):
Look up providers by their 10 digit NPI number, one per line. Can be used on its own or next to a search.
## `providerType` (type: `string`):
Individuals are people (doctors, nurses, dentists). Organizations are clinics, hospitals, pharmacies and groups.
## `addressType` (type: `string`):
What the state, city and ZIP code are matched against. Practice location is what most searches want.
## `maxItems` (type: `integer`):
The run stops after this many providers. Minimum 10. One registry search reaches 1,200 providers; bigger requests are split by type, state and ZIP automatically.
