Extract rich doctor profile data from Healthgrades.com using pure HTTP — no browser, no Playwright, no expensive proxies.

v2.0 Rewrite

Rewritten from the ground up (was: Node.js + PlaywrightCrawler, maxConcurrency 1):

Pure HTTP via httpx async — 8 concurrent requests
JSON-LD parsing (@graph → IndividualPhysician) for structured data
HTML fallbacks for name/phone when JSON-LD is incomplete
Retry with backoff for transient failures (429/5xx)
