Looks up licensed US healthcare providers in the NPPES NPI Registry — the national provider directory published as open data by the Centers for Medicare & Medicaid Services. Search by specialty, location and name, and get back one flat row per provider: NPI number, name and credential, specialties, state licence, practice and mailing address, phone and fax, and when the record was last updated. Covers both individual clinicians and healthcare organizations such as hospitals, clinics, labs and pharmacies. No account or API key needed.
Input
| Field | Default | Description |
|---|---|---|
specialty | Dermatologist | Specialty to search for. Everyday names work (Dermatologist, Cardiologist, Pediatrician) and are mapped to the registry's own wording; registry terms such as Family Medicine or General Acute Care Hospital work too. Leave empty to search by name and location only. |
location | Austin, TX | City, ST, a state name or code, or a 5-digit ZIP. Matched against each provider's primary practice address. Leave empty for a nationwide search. |
last_name | (empty) | Optional last name, or organization name when provider_type is organization. A trailing * acts as a wildcard. |
provider_type | individual | individual (clinicians), organization (facilities) or both. |
max_results | 20 | Maximum rows to return, up to 1200 per search. |
proxyConfiguration | Apify proxy | Optional. The registry answers directly, so a proxy is only needed if you want requests routed through one. |
At least one of specialty, a city or ZIP in location, or last_name must be
given — a state on its own is too broad for the registry to answer.

Every row also carries a raw field with the registry's own record, so nothing
is lost.
Notes
- Source: the NPPES NPI Registry public search service operated by the Centers for Medicare & Medicaid Services. This is US government open data and is free to use; no key, account or sign-in is involved.
- Organization rows fill
organization_name,authorized_officialand the address fields; the person-name and gender fields stay empty. Individual rows are the other way round. - The registry returns a maximum of 1200 records for any single search. To collect more, split the work by city, ZIP or specialty and run the actor once per slice.
- Records reflect what each provider last filed with CMS.
last_updatedshows how current a row is, andstatusflags deactivated NPIs. - Requests are paced about half a second apart to stay well inside the registry's fair-use expectations.
