What you get
Every record is a Florida health-care practitioner license, returned as flat columns ready for CSV or Excel:
- Identity & license — license number, full name, first and last name, profession, city and license status (Clear/Active, Null and Void, Retired, and more).
- Address & dates — full address of record, license expiration date and original issue date.
- Compliance flags — discipline on file, public complaint indicator and controlled-substance prescriber indicator.
- Physician profile (MD/DO) — primary practice name and address (street, city, state, ZIP), year began practicing, contact email when the practitioner provided one, specialty certification board and certification, number of specialty certifications, Medicaid participation, hospital staff privileges and other-state licenses.
You control depth: keep Fetch Full Profile Details on for the complete profile, or turn it off for a fast, listing-only pass.

Frequently Asked Questions
Which professions does this scraper cover? Every board and council in the Florida Division of Medical Quality Assurance license search — including medical doctors, osteopathic physicians, physician assistants, registered and practical nurses, advanced practice registered nurses, dentists and dental hygienists, pharmacists, physical and occupational therapists, chiropractors, optometrists, acupuncturists, massage therapists, clinical social workers, mental health counselors and many more. The profession is returned on every record.
What contact and address data is included? For physicians (MD/DO), the practitioner profile adds the primary practice address and, when the practitioner supplied one, a contact email address, plus specialty certification, hospital staff privileges and other-state licenses. All practitioners include the address of record and license dates when Fetch Full Profile Details is on.
How do I search? Enter one or more last names, and optionally narrow by first name, profession code, board, city, county, ZIP, license number, business name or license status. Each last name runs its own search, so one run can cover many people. You can also search by profession and county alone, with no name.
Can I search by license number? Yes. Enter the exact license number, for example ME122256, to pull a single practitioner.
What happens when a field is missing? Fields the source does not provide for a given practitioner are returned as null rather than guessed — for example email is null when the practitioner did not supply one, and specialty is null for professions that do not report board certifications.
Do I pay for empty runs? No. You are only charged for real practitioner records returned. A search with no matches costs nothing.
Are nurse and psychologist records complete? The Florida source redirects some nursing records to the national Nursys system and some psychology records to PSYPACT, so those subsets can be partial. All other health professions return the full profile.
