Upwork is gradually changing their page layouts so I am updating the actor to the new UI. Can you share the run ID with me to help me with that?
I have released a new beta version that fixes the issue with name and title, I am working to solve the other fields.

Seems like the run ID is invalid
The fix was only in the beta version, I have added it to the lastest version also. Can you check again?
