Export rural hospitals and clinics from the official USAC Form 461 table: site name, published email, phone, county.
No login. No API key. Not E-Rate school bids. Not Google Maps.
Emails and phones only when USAC publishes them — never invented.
One row per unique HCP number. FY2026 has ~4,300 unique sites (~96% email). All years combined: ~12,000 unique HCPs.

FAQ
Do you invent emails? No. A mailbox appears only when Form 461 prints one.
Is this E-Rate Form 470? No. Form 470 is schools and libraries. This Actor is Rural Health Care Form 461.
Why unique sites, not 90k rows? The open table is one row per application. Repeat filers would duplicate the same mailbox. We keep one HCP.
Do I need an API key? No.
