What you get by default: site name, published email, phone, city, county, funding year. This is not FCC Form 470 (schools and libraries).

1. Read the public USAC Form 461 table for the year and states you picked.
2. Copy site, contact, email, phone and county when they are printed.
3. Keep the first (newest) row per HCP number.
4. Write matching rows as the Dataset fills.

FAQ

Do you invent emails? No. A mailbox appears only when Form 461 prints one.

Is this E-Rate Form 470? No. Form 470 is schools and libraries. This Actor is Rural Health Care Form 461.

Why unique sites, not 90k rows? The open table is one row per application. Repeat filers would duplicate the same mailbox. We keep one HCP.

Do I need an API key? No.
