Export rural hospitals and clinics from the official USAC Form 461 table: site name, published email, phone, county.

No login. No API key. Not E-Rate school bids. Not Google Maps.

Emails and phones only when USAC publishes them — never invented.

One row per unique HCP number. FY2026 has ~4,300 unique sites (~96% email). All years combined: ~12,000 unique HCPs.

What you get by default: site name, published email, phone, city, county, funding year. This is not FCC Form 470 (schools and libraries).

Quick start

  1. Open the Actor in Apify Console.
  2. Keep 2026 and Missouri, or pick another year and state.
  3. Leave Only rows with email or phone on for a first CRM export.
  4. Click Start.
  5. Download JSON, CSV, Excel, or HTML — views Overview or Outreach CRM.

Max unique sites = 0 → no Actor-side ceiling. Free plan: 20 / run.

How it works

  1. Read the public USAC Form 461 table for the year and states you picked.
  2. Copy site, contact, email, phone and county when they are printed.
  3. Keep the first (newest) row per HCP number.
  4. Write matching rows as the Dataset fills.

This Actor copies data USAC publishes on a public open-data table. Names, emails and phones are personal data — your use must comply with CAN-SPAM, TCPA, HIPAA marketing rules where they apply, and any rules that apply to you. This is not legal advice. See Apify on the legality of web scraping.

Independent tool. Not affiliated with USAC or the FCC.

Local development

cd us-rhc-form461-leads
npm install
cp .actor/INPUT.json storage/key_value_stores/default/INPUT.json
apify run --purge

Use apify run (not npm start). Root input.json is merged locally; CLI KV input wins on key collision. Local storage/ is not synced to Apify Console.

npm test runs mapper and county tests (no network).

FAQ

Do you invent emails? No. A mailbox appears only when Form 461 prints one.

Is this E-Rate Form 470? No. Form 470 is schools and libraries. This Actor is Rural Health Care Form 461.

Why unique sites, not 90k rows? The open table is one row per application. Repeat filers would duplicate the same mailbox. We keep one HCP.

Do I need an API key? No.
