Issue looks strange as I just ran the actor with 100% input as yours and was successful. Probably a bug somewhere. Please when did you start noticing this and how often can you reckon?

i think i saw it first time today. we have a 6 records like that today in our system.

i'm wrong. The oldest record which we have is from 29th june: XFzmj1LUrx14NZk98 run id. We have 60 runs like that.
